Daily Duties at Sentara Healthcare:

Certified Case Manager for the Clinical Care Services Pilot Team. The Pilot team is an integrated team of Nurses, Patient Service Coordinators, and a Behavioral Health Clinician. Duties included: •Maintaining productive call volume in accordance with team standards •Demonstrate effective time management skills •Coach team members to be proactive for positive health habits. •Communicates member information appropriately to CM team, facilitates member behavior change and supports, identifies health barriers. •Document in process and outcome on time. •Be knowledgeable about and maintain availability of resources needed to answer client questions. •Demonstrate knowledge of member benefits •Follow up with clients to ensure appropriate treatment and measure progress to expected outcome. •Provide and receives constructive criticism to team members. •Maintain professional and positive relationships with all members of team •Manages environment changes in a positive manner. •Demonstrate a strong and consistent work ethic while anticipating and resolving problems efficiently and effectively. •Eliminates barriers to progress, understands link between process and business results at the unit level. •As a behavioral health clinician ensure integration into the medical arena to learn as much as possible about disease processes and how to manage them. •Maintain continuing education training to remain on the cutting edge of care and services. Progress milestones at Sentara. •Initially employed as a Case Manager responsible for providing care management services for the behavioral health team with specific emphasis on crisis intervention, concurrent care management, and intensive care management, coordination of services, quality assurance, and utilization review. •Established and maintained liaison with providers and contracted customers, as well as clients in service. •Assumed responsibility and accountability for daily assignments including coordination of work and quality of service. •Promoted to Team Coordinator in April 2005. Assumed responsibility, accountability, and provided leadership for daily clinical operations, coordination of work, quality and service, and supervision in SBHS Care Management Department. •Facilitated the work of assigned team members, sharing responsibility with other team coordinators in designated areas. •Participated in the work activities of assigned teams through providing assistance, serving as Care Management clinical/technical resource expert and participating in the work as a member of the team. •Contributed to the success of the SBHS through effective work coordination, management of resources, identification and implementation of identified opportunities, and participation in goal accomplishment. •Served on committee to develop electronic medical record for case management department at Optima Health Plan
