Facilitated life skill sessions for 20 mentally ill & developmentally disabled individuals Services individuals who were independent to semi - independent and ready to integrate within the community without the staff’s assistance Frequent telephonic case management to consumers about community resources & ADL skills - grooming, cooking, cleaning, shopping, social skills, etc. Document and charted service plans Reviewed and assessed weekly, monthly and quarterly service plans Attended utilization reviews when requested
