Daily Duties at Montefiore Medical Center:

Provided tele-nursing and medical services affiliated with the specific target population of congestive heart failure (CHF) and diabetic Health First insurance chronic members Caseload of 250 members Out of 250 cases touched on 90% of caseload on a monthly basis Prevent re-hospitalizations and emergency room visits Rapport with primary care physician and staff about member’s progress Patient education related to follow up care appointment, medical diagnosis and prescriptions Referrals to community programs related to members medical, psychosocial needs Promoted the sale of medical devices such as Life Alert to at risk members who may need emergent medical assistance Evaluated new members with primary care needs related to cognitive functions, medical diagnosis, ability and willingness to work, domestic, sexual, elderly, and financial abuse Correlated with external agencies who provided care to the client about the progress of members wound care and other skilled nursing needs Met with healthcare team about the progress of priority members Called clients per month to find out about progress according to priority of needs Improved upon quality of work as per performance evaluation from team leader Provided translation services when needed
