Daily Duties at Heartland Home Health Care :
Initial Assessment, assess goals to be met, assess progression to goals by teaching evaluation, reevaluation, and referral to a longer term care facility if needed due to not teachable or non compliant. Supervise nurse aids and collaborate with other disciplines on the case.
