I am a case manager who does the initial assessment or evaluation of a patient and his or her needs. I then formulate a plan of care and order the appropriate disciplines needed by the patient. I interact with them about the progress of their patients. I also see patients, administer IV's, teach patients how to take care of themselves for prevention of falls and infection. I take a proactive approach to my patient's care to keep them out of the hospital through thorough teaching of disease process, medication actions and side effects and I teach early signs and symptoms of complications to report. What any patient needs physically, I either teach the family how to do it, or I do it myself. But the patient is ultimately my responsibility as well as the out come of the care they received.
