Daily Duties at Holy Redeemer, Nazareth And St Mary Hospice And Vna:
Responsible to assess patient needs, implement appropriate care, teach patients according to individual care plan, for both Hospice and Home Health patients. Supervise Home Health Aids. Communicate with Physican, Nursing supervisor and team members of all relevant changes . Provide written documentation . Advocate for patients for adequate pain management and symptom contol and to promote a dignified death for Hospice patients.Assist in establishing and implementing goals for homebound clients
