Daily Duties at Blue Ridge Hospice :
Perform initial intake referrals, obtain physician order, validate insurance data, initiate contact with patient and families. Perform initial physical and psychosocial assessment and evaluate appropriateness for Hospice admssion using Medicare guidlines. Develop initial plan of care, explain Hospice program and benefits, educate patient and families on symptom management. Provide emotional support to patients and family members. Ensusre all documentaion is shared with Physician and Hospice team members. Initiate and maintain contact with physicians, hospitals and skilled care and assisted living facilities to provide information on Hospice program and education on end of life issues, pain management and symptom control
