Daily Duties at Aetna, Inc:
* Actively involved in team presentations for prospective clients by representing the Case Management Model, and demonstrating seamless collaboration between departments * Act as a resource and provide education regarding disease processes and treatment, so that members are able to make informed decisions and self-direct care as appropriate. * Accountable for the coordination of care for Aetna insurance members in a dedicated unit for a new national account. Responsibilities include facilitation and coordination of home care and infusion services, hospice care, and inpatient rehabilitation utilizing both internal policies and Milliman Care Guidelines. * Utilize critical thinking skills in a collaborative process to assess, plan, implement, coordinate and evaluate options and services to facilitate appropriate healthcare outcomes for member. * Identify and address gaps in care by collaborating with an interdisciplinary team that includes Leaves and Disability; Disease Management; High Risk Maternity Case Management; and Behavioral Health Case Management. * Provide comprehensive healthcare management to facilitate the delivery of Best-in-Class healthcare with the promotion of cost effective outcomes. * Act as a resource to co-workers, and participate in new hire training including education regarding case management processes, care plan development, and identification of member needs.
