Managed care requests based on patient's Medicare replacement coverage via Humana. Directed members to in network providers and services.Interacted to ensure compliance with Humana Medicare replacement allowed services. Worked with Humana staff to review unusual cases for possible out of network coverage. Kept track of members in hospital or skilled nursing facilities for appropriat level of care and consulted and coordinated with Humana field nurses to facilitate discharge and establish home care, follow-ups, etc.
