Daily Duties at Dept. Of Veterans Affairs:
Provide intensive case management services for veterans in the HUD/VASH program Responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities, and the VA in establishing appropriate referral sites for HUD/VASH placements. Conduct routine suicide risk screens, comprehensive assessments, and collaborative safety plans Coordinate care, including serving as an advocate on behalf of veterans to ensure comprehensive service deliver, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Provide on-going therapy and counseling to veterans in the program Conduct home visits to ensure health and safety of veterans in the program Assess homeless veterans to determine eligibility for HUD-VASH program * Participated in Literature and Medicine Program
