Coordinated/development Resident Service Plans, assuring resident needs were addressed and cost was accurately assessed. Conducted weekly audits of documentation, to include weekly pharmacy review. Conducted mock surveys to assure community survey readiness, areas of concern were immediately addressed. coordinated care conferences, established level of care changes during the daily QA meeting to assure accurate level of care. Liaison for the community, resident’s family members and outside resources. On call 24/7 to provide nursing accessibility to staff, family members and emergency admissions.
