Daily Duties at Disability Action Center (Dac):
At Disability Action Center I network and interface with multiple agencies to assist client’s needs and link them with different resources. I also work as an advocate and job coach. My duties include: * Being an advocate / diplomat between the person in the home and Medicaid to assure services are provided and maintained. * Monitor and maintain paperwork with Medicaid and monitor services with people utilizing In-Home-Health Care. * Discuss how the people in the home’s services are progressing; find out what other services they need. * Keep up with different services offered to the aged and disabled population. * Maintain relations with other agencies. * When a person calls in needing services of any type, I assist them in connecting with the services needed. * When a person calls with a problem, I assist them in determining what services they need and which agency to connect them with and, when necessary, assist then in filling out the paperwork, whether it is with moral support, writing, online help or reading and translating into common terminology
