Responsible for direct case management and assessment of homeless families in a faith based shelter environment. Provided comprehensive services in assessing family needs and documenting information in order to make casework decisions. Communicated and coordinated service provisions with collateral community agencies and program management. Placements exceed 80% in transitional or permanent housing.
