Daily Duties at Rels Home Corp:

Provided direct patient care under direction of nursing and medical staff. Assigned nurses and physicians to cases for assessments. Assigned home health aide to cases. Review Policy for medical insurance companies. Assigned claims to Medical/Medicare, Blue Cross, Aetna, Cigna Set up in-services for brand new patients patient’s billings. Prepared and presented Nursing Care Plan at Care Conferences. Maintained patient records including Minimal Data Sets (MDS) according to Medicare guidelines. Provides ongoing, objective assessment of the injured or chronically ill patient, Formulates a treatment plan to maximize the quality of care while minimizing the costs. Coordinates services and promotes communication to maximize recovery. Emphasis continuity of care, effective communication, and coordination of appropriate health care services for patient(s). Ensures senior adult receives the best services and care possible for their needs Monthly visits to assure quality care to prevent complications and/or problems Medical billing coding making sure health/ insurance companies and patients is being billed correctly
