Provided physical, emotional and spiritual care, along with practical support to Hospice patients of all ages and their families, following the principles of hospice philosophy. Supervised and coordinated the care provided by home health aides/homemakers, LPN’s, volunteers, contracted agencies, and other ancillary personnel involved in the care of the hospice patient. Organized the provision of nursing care to patients through the process of assessment, planning, intervention, and evaluation.
