Daily Duties at Asera Care Hospice:
Admits, discharges, and revokes. IDT was a weekly event reviewing all current patient status'. Home visits, pronouncements, arrangements of funneral needs, bereavement follow-up and all immediate needs for the patient, family and friends. Assessments, planning implementation and evaluation phases of the nursing process. Communication with patient, family, friends of patient, supervisors, pharmacists, hospice doctors and primary care physicians were always a constant occurrence.
