Daily Duties at Visiting Nurse Association:
Under the general supervision of the patient care supervisor I was directly responsible for the care given to a group of patients. This includes implementation of case management systems, assessment of patient needs, developement of patient care plans, scheduling of patient and team assignments, administration of nursing care, evaluation of nursing care and supervision of non-professionals in the delivery of patient care in the home care setting while maintaining productivity standards. In this role, the nurse case manager is responsible for positively impacting: clinical outcomes, patient care plan goals, appropriate utilization of the agency's financials and human resources, and patient satisfaction.
