Provide physician practices and their clinic with a day-to-day operational evaluation. This is accomplished by: • Conducting a comprehensive evaluation and assessment of clinic operations, including physicians, managers, staff, patients and their families • Working with Physician, patients and their families, clinic staff and management in developing strategic operational direction e.g. develop and implementation of the “Patient Centered Primary Care Home Model of Care” • Identifying what is operationally working well and what areas need improvement • Assist in staff development and plans for continuous improvement • Assist Physician leadership and management, to quantify and measure improvement in care and service delivery, e.g. adherence to the guiding principles of the Patient Centered Primary Care Home Model of Care
