Home Care to clients/patients with a referral from a primary care physician or inpatient referral Arrange home care for specific needs adhering to the diagnosis and preventive care Education to the patient and families/significant others Participate in teaching seminars including an audience of medical personnel Community resources and education concerning disease management Communication with the insurance companies for coverage
