Case Manager inpatient Assessment of the patients admission, length of stay, discharge planning,education and discharged appts in place Rounds with CEO of the insurance company affilated with the medical center Case management for the Congestive Heart Failure program Implementing resources for re-admissions in high risk populations Participating in community events Providing educational booths on disease prevention and wellness
