Made rounds with cardiologists pre and post heart failure and transplant patients Educate families and patients concerning transplant Discussion and review of the Transplant booklet ie medication, activities, diet, signs and symptoms of rejection of the organ Available to the patients and other phtsicians concerning patient care Education community and physicians about transplant Seminars, presentations, and panel discussions with Dr Kirk Adams(CHF MD) Director of Cardiac Transplant Dr Patricia Chang also made rounds with her on post transpalnt patients; reviewed lab work, biopsy results, ECHO, Radiology results and see patients in clinic and on the floor. Used clinical skills as RN on the floor and in the clinic. Research team with Dr Chang Interaction with Pharmaceutical companies Weekly meetings with cardiologists, CHF MD, social worker, Psychologist, physical therapist, financial advisor, and medical assistant(collaborative meetings) and also key speaker if requested such as lab personnel Pediatric patient coordinator with peds department Assessment of patients in clinic Pre/Post coordinator for pediatrics, same routine as adults except less patients Attended transplant support meetings
