Daily Duties at Axis Healthcare:

Health Coordinator • Case management for people with mental and physical disabilities. Majority of case load includes members with a diagnosis of brain injury. • Assisting members with management of complex health concerns. • Collaborating with team nurses, social workers, therapists and mental health professionals to individualize care plans designed to meet member’s goals. • Followed member’s medical management and made recommendations to reduce hospitalizations and emergency room visits. • Utilize creative problem solving to help control healthcare costs while providing patients with services they need. • Guide members to available housing. Assist with tours of housing providers as well as establishing an appropriate follow up plan individualized to meet member’s goals.
