Daily Duties at Evercare/United Health Group:
In LTF had panel of ~80 to 90 patients and evaluated acute and chronic issues and provides and adjusted treatments and meds; worked with interdisplinary teams, family, hospice,PCP and consultants to give pts best possible care. Gave inservices to staff on regular basis. All info placed in computer.
