Daily Duties at Evergreen Woods Health & Rehabilitation Center :
Provide discharge planning and fulfillment, including home healthcare and durable medical equipment provisioning. Screen and evaluate new admissions. Provide and explain resident personal and property rights, funding options (Medicare/Medicaid) , living wills, et al. Meet with admissions, nursing and medical, et al. in planning social services for residents. Participate in care plan meeting with dietary, activities, and MDS staff. Implement resident ongoing healthcare and wellness programs and services. Participate in community planning to enhance program and service provisioning to residents. Provide resident socialization conferencing and services. Institute, plan, and conduct Bible study and discussion program to meet residents’ personal challenges and spiritual needs. Review program and services outcomes with resident families. Coordinate logistics for resident dental, hearing, and eye care. Facilitate resident transportation to off-site appointments. Coordinate resident placement in other facilities including assisted living, personal care homes, et al. Perform administrative and documentation requirements. Review policies and procedures and propose revisions and improvements.
