Care Manager - Case Management Department - Triage Nurse/Utilization Review - (Oct. 2003-Jan. 2008) •Maintained Routine interaction with patients, physicians, internal departments including Enrollment, Fulfillment, Customer Service, Branch Operations staff, Support personnel, Information Systems, other team members, Clinical Manager and Regional Director of Operations or V.P of Operations. •Provided telephonic interaction for enrolled patients utilizing nursing skills to triage, assess, evaluate, inform, instruct, and monitor. •Coordinate network participation, specialty networks, Disease Management providers and patient transfers to alternative levels of care using knowledge of benefit plan design. •Conduct admission reviews. •Documented care to achieve program goals and outcomes. •Interacted and delegated on a daily basis. •Educated and empowered cancer patients and their families. Intake Coordinator - Pre-Cert/Authorization Department (Sept. 2000 – Oct 2003) •Provided authorization for inpatient/outpatient procedures per health plan and company policies using cancer disease criteria and Interqual Milliman & Roberts Guidelines (Medicare guidelines). •Audited coding on medical charts to insure billing compliance. •Reviewed and processed all Home Health, DME and orthotics/prosthetics authorization requests. •Verified eligibility and entered new patient information into the Integrated Care Management System. •Reported to Quality Oncology and Plan Medical Director any out of network/out of guidelines referral request. •Trained other intake nurses in the authorization process. Interacted with all members of the organization to meet company’s goal to promote and provide cost-effective and efficient cancer care.
