Daily Duties at Grady Health System:
Managed patient base with wide-ranging needs in Urgent Care Center of large public hospital serving Fulton-Dekalb County area. Oversaw patient care lifecycles, including establishment of service delivery and discharge plans, as well as treatment follow-up. Performed psychosocial assessments and processed patients for service referrals. Convened treatment teams in multidisciplinary setting for design of individualized patient care plans. Provided social serve referrals to medical patients, as well as homeless and abused individuals. Offered counseling in crisis intervention and conflict resolution. Advised on prevention of sexually transmitted diseases, and guided on issues pertinent to nursing home care.
