Daily Duties at Southern Home Health Nursing:
I was an RN Case Manager for thirty eight patients. I was responsible for admitting and re-certifying patients to home health care. This included the number of visits each patient would need each week and type of visit. I was responsible for making supervisory visits with the CNAs to insure the correct care was being given. I called the doctor when needed for medicine refills, or changes in status. I also determined based on my assessment would have PT/OT/ST to come in.
