Daily Duties at American Home Patient :

Entered company as field case manager making home visits then moved to Clincal Supervisor managing team of nurses and home health aides for approximately 1 year. Started and organized the position of Intake Coordintor responsible for receiving and processing all new referrals, entering them into McKesson system then coordinating start of care with Clinical Supervisors. Monthly statitics regarding number of admission, non-admits, referral sources, etc and reporting to Director of Nursing. Since beginning this position referral process was streamlined which referral sources appreciated and number of referrals continues to increase.
