Case Managed both Homecare and Hospice patients, 10-30 average case load, home visits including admissions, recerts, resumptions and discharges. Electronic documentation via McKesson Homecare System. Interdisciplinary Care Coordination. Team Meetings and Care Planning. Experienced with venipunctures, wound/ostomy care including wound vac therapy, IV site maintenance and administration, and GT/PEG tube care.
