Daily Duties at Stein Hospice:
Responsible for coordination of care for caseload of hospice patients. Duties include weekly assessments, triage visits as needed, monitoring of medication, notifying physician for changes, providing care to keep patients comfortable physically, emotionally, and providing referrals to other disciplines as needed. Perform admissions of new patients to the hospice organization. Perform cerifications for patients as needed by Medicare to determine need for hospice care. Hold IDT meetings other disciplines to discuss and evaluate patient care and needs for changes in plan of care.
