Responsibilities include coding inpatient and assisting with outpatient hospital services, abstracting information from the medical records, and coding accurately and effectively to adhere to coding guidelines. Responsibilities include abstracting, analyzing, concurrent coding, and custodian of medical records, deficiencies, and completing medical records. Duties and responsibilities include quality assurance reviews for blood, procedural, moderate sedation reviews, and reviews for each discipline to ensure discharge summaries and orders are signed and completed within the thirty-day requirement. Maintain a low monthly delinquency rate of seven percent or less. Responsible for keeping a log of all discharges, discharge days, and average case mix index for each month within the fiscal year calculating the Case Mix Index for discharges, calculating the Average LOS, and monitoring timeliness of history and physicals. Duties include finalizing DRG and ensuring documentation supports principal diagnosis and the ability to capture all complications and co-morbidities. Responsible for running monthly SQL reports tracking the number of admissions, discharges, mortalities, top 10 Diagnostic Related Groups for each month; and gathering the Case Mix Detail for the month. Developed incentive program for nursing staff and medical staff working on the floor to boost moral and help reduce deficiencies. This incentive program proved beneficial to our medical records department and boosted the moral of the staff. Maintain a low history and physical and deficiency rate for physician’s signatures. Conduct weekly reviews to ensure physicians are documenting the reason for the patient’s admission into our facility and querying the physician when additional documentation is required in the medical record. Analyze and maintain all deficiencies within our department. Assemble every medical record within the department. Provide direction for one part time employee to deliver and pick up physician folders for signatures and deficiencies. Responsible include the calculating of total patient days from our host hospital and non-host hospital. Work closely with Case Managers and Physicians to ensure appropriate documentation is documented in the patient’s medical record to clinically capture diagnoses and procedures during the patient’s hospital stay.
