Daily Duties at Dshs Home & Community Services:
Providing long term care case management for clients of state funded services. Monitored service delivery including oversight of payments using state invoicing system and state required software programs. Case management, information and referral, liaison to client/family/providers/agency. Monitoring Medicaid eligibility (financial and physical) of clients. Required extensive knowledge of state and federal funding programs, local and state community agencies and programs available to clientele. Required extensive knowledge and understanding of WACs and RCWs pertaining to long term care facilities
