Daily Duties at Institute For Family Centered Services:

Develop and coordinate the Person Centered Plan; as well as coordination and a variety of interventions. Provide service coordination activities within the established PCP, referral linkage, skills building, supportive counseling and input in to the PCP modifications. Provide the required contact/daily, reports, plans of service or treatment, court reports on time in a form that meets the needs of the family, collateral and requirements. Provide 24/7 on call support for all assigned families. Participate and support peer development process by respectfully addressing areas of concern for peer professional development and team performance issues.
