Daily Duties at Turning Point Aip Program:
Case managed 54 women to secure housing, A & D Treatment, and other resources for their release from prison; Identified needs of client population and creates intervention to address them; set up and attended PO reach-ins for each client in the program; provided information and skills from evidenced-based curriculum to address criminal risk factors; advocated for services that best serve individual clients; reviewed police reports, PSI, and disciplinarian records on each client and share this information with the Treatment team; incorporated expectations from the Department of Corrections staff, Parole Officers, and the Treatment Team to formulate an individualized aftercare plan for release which becomes a part of the transitional leave expectations; communicated with DOC staff, POs, housing providers, and treatment providers to ensure a continuation of care through community supervision and programs upon the client's re-entry into the community; provided bi-weekly updates on clients' progress and compliance in treatment to the AIP committee; provided inmates with an ability to contact mentors, support people, and healthy family members by telephone and through PO reach-ins; documented contact with and for clients for their Treatment file.
