Want to be part of a team, that works together to develop, implement, manage and evaluate patient care needs. To work with patients, to first establish their goals, then directing nursing care for patients in home and residential settings so that their goals are met and the need for hospitalizations are kept to a minimum by: 1) Communicating and collaborating with patients, families, physicians and other community supports 2) Teaching on medical issues and/or mental health topics such as stress reduction, coping with depression or anxiety, reality based orientation and medication compliance.
