CAREER SUMMARY As Director of Health Services, has proven to be an effective leader in building, coaching, and coordinating a high performing team of health care professionals. Utilizing her broad experiential level in nursing care, is responsible for hiring, orienting, scheduling, and coaching a team comprised of RN's, CNA's, Social workers, Chaplains, and Volunteer Coordinators in the delivery of "exemplary hospice care" to all patients under her teams care. Director of Health Services: * Develops and delivers In-Services for staff and referral sources to educate and increase viability of offered services. * Responsible for admission, certification, and discharge processes & documentation per federal regulations. * Facilitates weekly IDG meetings with staff & Medical Directors to ascertain Hospice appropriateness. * Performs monthly chart audits, QAPI requirements. * Conducts admissions, routine nursing visits, and assumes on-call responsibilities as needed * Reviews billing for medications, DME's, and treatments for accuracy and approval. HOSPICE CASE MANAGEMENT: - Developed, implemented, and managed comprehensive plans of care for average case load of 14 patients * Conducted at minimum weekly home visits for patient assessment, with follow-up phone calls for reinforcement of health teaching/plan of care. - Collaborated with patients primary care physicians, Hospice Director of Pharmacy, & local pharmacies - Delivered extensive "Health" teaching" to patient & family of disease progression and available agency support systems. SURGICAL EXPERIENCE: - Experience in multiple operating room settings as an O.R. Manager, Circulating nurse, and Scrub Nurse. - Developed internship programs, in addition to interviewing, hiring, training, and managing OR staff * As O.R. manager, managed Hospital/Union requirements, developed and managed annual operating budgets, and implemented computerized scheduling systems. CLINICAL STAFF DEVELOPMENT & INSTRUCTIONAL DELIVERY : * Interviewed, oriented, evaluated, and insured clinical competencies for all Surgical Services Staff.
